In a double-blind, placebo-controlled trial, 154 patients with chronic hepatitis C who had not responded to interferon-based therapy were randomized to 420 mg silymarin, 700 mg silymarin, or placebo three times daily for 24 weeks. After treatment, only 2 participants in each of the three groups met the primary outcome of a normalized or substantially reduced ALT level, and the mean ALT decline did not differ significantly between groups. Higher-than-customary doses of silymarin did not outperform placebo on this measure.
