Sustained-Release Melatonin - Efficacy in Insomnia

Prolonged-release melatonin improved sleep quality, morning alertness and sleep latency in adults 55 and over

A randomized, double-blind, placebo-controlled trial in 354 primary care patients aged 55 to 80 found that a prolonged-release melatonin formulation produced significant, clinically relevant improvements in sleep quality, next-day morning alertness, sleep onset latency and quality of life compared with placebo.

Human RCT

Sustained-Release Melatonin - Efficacy in Insomnia

Meta-analysis: prolonged-release melatonin eases sleep onset latency, with a larger effect in adults 55 and older

A systematic review and meta-analysis of 22 studies (4,875 participants) found prolonged-release melatonin produced small-to-medium improvements in subjective and objective sleep onset latency and sleep efficiency versus placebo, with a notably larger effect on sleep efficiency in the subgroup aged 55 and over.

Meta-Analysis

Sustained-Release Melatonin - Efficacy in Insomnia

Review: prolonged-release melatonin is designed to mimic the body's own multi-hour secretion pattern

A review of melatonin physiology found that endogenous melatonin secretion spreads over 8 to 10 hours nightly, and describes how prolonged-release melatonin formulations were developed specifically to mimic this natural secretion pattern rather than the rapid rise and fall of immediate-release forms.

Prolonged-Release Melatonin - Long-Term Safety & Tolerability

6-month trial: prolonged-release melatonin maintained benefits with no rebound or withdrawal effects

A randomized, placebo-controlled trial following 555 insomnia patients for up to 6 months found sleep improvements with prolonged-release melatonin were maintained or enhanced over time, with no signs of tolerance and no withdrawal symptoms or rebound insomnia detected on discontinuation.

Human RCT

Prolonged-Release Melatonin - Long-Term Safety & Tolerability

Postmarketing study of 653 patients: sleep quality and morning alertness improved, rebound insomnia rare

Melatonin & the Aging Sleep-Wake Cycle

Endogenous melatonin production declines with age, flattening the nighttime peak

A study measuring plasma melatonin across age groups found older adults showed a flattened circadian profile with a suppressed nocturnal peak compared with younger subjects, part of a well-documented age-related decline in the body's own melatonin secretion.

Melatonin & the Aging Sleep-Wake Cycle

Systematic review confirms a dose-response relationship for melatonin supplementation in older adults

A systematic review of 16 studies in adults aged 55 and older found that exogenous melatonin administration raised melatonin levels in a dose-dependent fashion, with a higher elevation seen in older adults than in younger adults given the same dose.

Melatonin & the Aging Sleep-Wake Cycle

Network meta-analysis: melatonin shows more benefit for older adults than younger adults

A network meta-analysis comparing sleep medications across 111 trials and nearly 26,000 participants found that, as a supplement, melatonin had little effect on younger adults but still showed a certain benefit for older adults' sleep outcomes.

Melatonin for Insomnia in Older Adults

Review of 12 studies: low-dose melatonin improved sleep quality in selected elderly insomniacs

A systematic review of double-blind, randomized crossover trials in elderly insomniacs (mean ages 65 to 79) found sleep latency decreased significantly in the majority of studies, with low doses of melatonin - some using slow-release forms - improving initial sleep quality without early-morning sleepiness.
